Application Information Required

There are ten steps to complete using our online application.

Step one - Requests the following information;

Name - Phone Numbers - Email Address - Username - Password

On this page you must create your personal username and password, this will enable you to login at a later date to edit or add information to your application.

Step Two - Requests the following information;

Your Sport - Your Club - Date of Birth - Occupation - Number of Years Calgary Resident - Address - Your Parents Information.

Step Three - Requests the following information;

Budget from: September 1, 2010 - August 31, 2011 (Academic Year) Income & Expenses.

Step Four - Requests the following information;

Outstanding Loans or debts - NSO Reserve Fund Information - Vehicle Information.

Step Five - Requests the following information;

Summary of Top Athletic Performances: (details of past three years) International Rank/Performance.

Step Six - Requests the following information;

Summary of Top Athletic Performances: (details of past three years) national Rank/Performance.

Step Seven - Requests the following information;

Summarize Athletic Performance Aspirations/Projections For Next Year.

Step Eight - Requests the following information;

Are you registered in a post secondary educational institution - If you are registered in a post secondary institution please provide proof of registration - Need For Assistance and Special Circumstances The expenses should include actual expenses paid by the athlete in order to be part of the Canadian National Team. If you are asking for additional funds for travel and training camps, an itemized list of expenses and/or requirements should be enclosed.

Step Nine - Requests the following information;

Letter One - Attach TWO Letters of Reference (National/Provincial Coach and Administrators) and any other information you may feel may be relevant.

Step Ten - Requests the following information;

Letter Two - Attach TWO Letters of Reference (National/Provincial Coach and Administrators) and any other information you may feel may be relevant.
