Part A - Sport Ability (Weight 40%)
List the nominee’s involvement and awards/achievements. Include awards and results of the nominee’s participation pertaining to the sport being recognized OR for all sports if applying for the 3rd Year Awards including ALL SCHOOL & CLUB SPORT INVOLVEMENT. (Examples to include are tournaments, awards, recognitions, position played, captain etc…)

Part B - Academic Achievement (Weight 30%)
Use chart below to list student’s achievements and academic awards received. (Examples to include are honour roll, distinction, scholarships etc…)

Provide a current school transcript and a copy of the student’s most recent report card from the school.

Part C - School Involvement (Weight 25%)
Use chart below to list examples of Citizenship, Sportsmanship and Leadership. Please indicate specifically the time commitment made by the student for each point. (Example: one weekend, once a week for 3 months, once a month for the last year. Include things such as team trainer, planning committees, grad committee, band, drama productions, open house tour guide etc…) and the main adult supervisor contact information. Please list all of the school athletic teams and involvement the candidate has been in over their three years of high school (can also include those teams they plan to join in this, their grade 12 year).

Part D - Community Involvement (Weight 5%)
Use chart below to state examples of UNPAID volunteer community involvement beyond the school level within the last 2 or 3 years. This may include participation in clubs or community athletic involvement. Please indicate specifically the time commitment made by the student for each point (example: one weekend, once a week for 3 months) and the main adult supervisor contact information.

Part E - Student Declaration of Consideration
Student write up (anywhere from a half page to a full page) as to why you deserve this scholarship…i.e.family responsibility, achievements, future education plans, sportsmanship, teamwork, leadership, etc.

Part F - Coaches Statement on the Nominee
This should be a typed letter (one page maximum) from the head coach, highlighting the nominee’s assets
